The Government of Madhya Pradesh is working very hard to protect the interest of employees and promote their welfare. It came up with a policy known as the Mukhyamantri Karamchari Swasthya Bheema Yojana, where in the employees and staff of various Private and Public Enterprises in the state will be given suitable treatment and compensation on case to case basis.
It was started by the then Chief Minister Shri. Kamal Nath on 4th January 2020.

- To protect the families from financial pressure and liabilities due to the unforeseen health hazards and emergency situation in Madhya Pradesh.
- To ensure that people from various Public and Private enterprises will get access to the quality health care services.

- To make sure that quality and dedicated healthcare facilities and protection is offered to the employees of Public and Private Companies.

CATEGORIES OF EMPLOYEES AVAILING BENEFITS UNDER THE MUKHYAMANTRI KARAMCHARI SWASTHYA BIMA YOJANA

FEATURES OF THEMUKHYAMANTRI KARAMCHARI SWASTHYA BIMA YOJANA
The features of the Mukhyamantri Karamchari Swasthya Bima Yojana are given below as follows:
- This scheme was launched on 4th January 2020 led by the then Chief Minister CM Kamal Nath.
- This scheme was created to carry out the Compulsory Healthcare facility using the Secondary and Tertiary Health Care Facilities in the state.
- As per this scheme more than 11 Lakhs 60 thousands functionaries and employees will get benefitted.
- We believe in offering the medical facilities up to INR 5 Lakhs per annum and up to 10 Lakhs per annum for the treatment of the serious medical disorders in a family.
- Providing the OPD (Out Patient Department) facilities to all the employees in order to give them good treatment.
- 50% reimbursement of the rentals of the house and other treatments.
- The limit for treatment for longer duration is INR 25000 in a year.
- Health card will be issued for the working employees and the retired employees of the Government of Madhya Pradesh.
- The Medical Care for the self and families of the ESI Hospitals and Dispensaries are carried out.
- The entire facility available under this scheme is cashless in nature.
- On medicines the family member will get the discount as well as the waiver till INR 10000.

ELIGIBILITY CRITERIA FOR GETTING REGISTERED UNDER THE MUKHYAMANTRI KARAMCHARI SWASTHYA BIMA YOJANA
There are eligibility conditions for a person to become the member of the Mukhyamanthri Karamchari Swasthya Bima Yojana given below as follows:
- The employee must be working or retired employee of the Madhya Pradesh government.
- Age of the employee must be above 20 years.
- Employee must have the ID card to prove his name/department details.
- The families of the employee are eligible under this scheme.
- Employees of Municipal Corporations and Public Welfare Departments are eligible only if they get letter of consent from the respective boards.
- Disabled people are eligible to become the member under this scheme.
- It covers employees working in Rural/ Semi Rural and Urban areas in the state.
- Employees must have a passbook from a recognised bank situated in the state of Madhya Pradesh.
PROCEDURE FOR APPLYING UNDER THE MUKHYAMANTRI KARAMCHARI SWASTHYA BIMA YOJANA
The procedure available under the under the Mukhyamanthri Karamchari Swasthya Bima Yojana given below as follows:
- The employees must fill up the form as per the instructions.
- The employees must fill in all the details and attach the form and give it to the Village Development Officer/Block Development Officer.
- Xerox of all the Admission form, Case Document, and other documents like the Aadhar, Pan Card and others should be attached immediately.
- The Village Development Officer will forward the application to the District Health Officer.
- The DHO will forward the document to the State Level Technical Committee and they will scrutinise the application.
- They will ask the application about any document they require and the applicant must give the concerned form within 7 working days.
- The State Level Committee will monitor the application and approve the application.
- The applicant needs to wait for 30 days to get the enrolment under this scheme.
DOCUMENT REQUIRED FOR MEMBERSHIP INTHE MUKHYAMANTRI KARAMCHARI SWASTHYA BIMA YOJANA
The documents required in order to be a member of the Mukhyamanthri Karamchari Swasthya Bima Yojana given below as follows:
- Birth Certificate verified by Gazetted officer.
- Residential Certificate showing the address proof.
- Passport Size and the Stamp Size Photos.
- Driving License Xerox of the employees.
- Bank Account must be opened in any bank in Madhya Pradesh.
- Certificate for Differently-abled, Ex-Serviceman and others.
- Employee ID card showing the name and department details.
- Consent letter from the HOD if the employees is from Quasi -Government, Semi-Governmental establishment or working in any Municipal Corporations in Madhya Pradesh.
- Nativity Certificate Xerox copy signed by the gazetted officer.
- Bank Passbook showing the First/Last page of the passbook giving particulars about the account.
- Voter Id, Aadhar Card, PAN Card, Ration Card Xerox displaying the family members details signed by the gazetted officer.
- Affidavit of the notary to show any name change done.
